List of all current medications and doses
Your personal and family medical history
List of any current or past medical providers

Telehealth Appointments

You will receive a text message with an invitation to your visit 5 minutes before your scheduled visit time
The invitation will include a link to Hippo Health, which is a secure platform for video visits
Use Chrome as your browser

Please note: Due to state licensing requirements, you must be physically located in the state of Colorado during your telehealth visit.
