What You Will Need To Bring

Please bring the following with you to your appointment:

  • Photo identification card or driver’s license
  • Health insurance card (if applicable)
  • List of all current medications and doses
  • Your personal and family medical history
  • List of any current or past medical providers

Telehealth Appointments

  • You will receive a text message with an invitation to your visit 5 minutes before your scheduled visit time
  • The invitation will include a link to Hippo Health, which is a secure platform for video visits
  • Use Chrome as your browser
